Fraud Prevention

Any person who reasonably believes that a claimant has provided false or misleading information, or has submitted documentation that was fraudulently created or altered, is encouraged to report such information through the submission form below.

Reports of suspected fraud will be reviewed in accordance with the applicable Fraud Prevention procedures. The claimant who is the subject of such allegations will be afforded an opportunity to respond. If a dispute remains after the claimant has responded, or has been provided a reasonable opportunity to respond and has failed to do so, the matter may be submitted for determination pursuant to the applicable dispute resolution procedures.

To facilitate a thorough review and investigation, please provide as much information as possible regarding the suspected fraudulent activity, including, to the extent known:

• The nature of the alleged false, misleading, altered, or fraudulent information or documentation;

• The date(s) or timeframe during which the activity occurred;

• The identity of any individuals involved;

• The basis for your knowledge of the activity; and

• The identity of any other individuals who may possess relevant information.
