Use this form to request that an extension be granted for your claimant(s)’ executed Opt-In & Release forms. As a reminder, the due date for the Opt-In & Release form is thirty (30) calendar days after the Opt-In & Release was sent to the claimant. If additional time is needed, you must use this form to request it.
Upon receipt of a completed extension form, ARCHER will seek approval for a release extension and, once a response is received, will notify your firm.
