If requested by a Participating Firm via this form, Representative Authority Affidavits will be populated based on the information provided in the completed Representative Authority Schedule (Exhibit F). Accordingly, all data points requested in the Excel spreadsheet must be provided. Failure to provide these data points, or providing inaccurate or incomplete data, may result in processing delays.
It is the Participating Firm’s sole responsibility to ensure that the Exhibit F – Representative Authority Affidavit is provided within thirty (30) days after the applicable Participation Deadline. Additionally, the submission of the Exhibit F – Representative Authority Affidavit does not eliminate the obligation to ultimately obtain and provide the required authority documentation.
